Baker, West Virginia

Serving as the corporate location since 1981, our baker location provides quality, affordable care in primary and behavioral family medicine as well as family dental services. This site also offers an on-site pharmacy services and diagnostic x-rays.

Wardensville Medical

Wardensville, West Virginia

This location provides two primary medical care providers as well as an on-site pharmacy.

Romney Medical

Romney, West Virginia

Primary and behavior family medicine services are offered at this location.

E.A. Hawse

Mathias, West Virginia

This location has been providing primary medical and dental services for the Mathias community and the neighboring area since 1998.

Grove Street Health Center

Petersburg, West Virginia

Our Grove Street office offers primary and behavioral family medicine services and dental care services. An on-site pharmacy is available, as well as a nutritionist and diabetic educator by appointment.

Hardy County Medical

Moorefield, West Virginia

Primary and acute medicine are offered at this location. Walk-ins are accepted, but appointments are recommended. Hardy County Medical also serves patients during 2nd shift (up to 9pm).

School-Based Services

Students who participate in the health center must complete a parental consent form prior to accessing care.
